Breast Augmentation

Small breast tissue or eviscerated breasts after childbirth and breastfeeding is a real problem for women in terms of self confidence. Placement of breast implant is the best option for life time natural and youthful appearance. Breast augmentation surgery is carried out by the gel-drop (anatomic) or round shape of the implants under the breast or under the muscle.

Surgery takes about 1-1.5 hours and since it is done under general anaesthesia, no pain is felt during surgery. After surgery, there may be slight pains. These pains will go away with simple painkillers. During surgery, there are no obstacles for you to return our daily life within 5-7 days.

Silicone Breast Augmentation

Silicone implants have been used for breast augmentation for more than 60 years. There is a tremendous improvement in implant technology that makes the silicones even more safe than ever. We use FDA approved implants from Allergan, Mentor or Polytech. All of these implants have warranties against manufacturing flaws. There are round or anatomical implants. Now implants with double gel consistency are available for more natural outcomes. The implant may be placed under the chest muscles or just behind the breast tissue depending on the patient’s physical properties. This is a short surgery with small scars and a short down time.

Breast Reduction

Excess skin and fat tissue causing breast to be big are removed. Thus both breast size becomes smaller and breast becomes lifted. Surgery is carried out under general anaesthesia, no pain is felt during surgery. In the aftermath there is no pain, too.

In breast reduction size depends on the person’s deformation. Surgery takes approximately 2-3 hours and 5 days later person can return to daily life.

As for breast lift, there are suture lines from nipple towards underside in a straight line like a lolipop. However, these suture lines become uncertain in time. Surgery takes approximately one hour. Almost no pain is felt during surgery and returning to daily life occurs within 3-5 days.

Excessively large breasts are not only an aesthetic problem but unhealthy for the neck and shoulders. There are many techniques defined for making breasts smaller, but the best technique is the one that fits the patient's individual anatomy and expectations. As a general rule, the larger the breasts, the more the scars. Scar patterns may differ from one that is only around the nipple-areola complex to a classical inverted T-shape. For some pubertal breast enlargement cases laser liposuction may be a good alternative to open surgery with skin excision, as this technique leaves almost no scars. A mammography is advised before the surgery to screen for malignancies and also three months after to provide a baseline image for future mammographies.

Breast Augmentation with Mesenchymal Cell Enriched Fat Injection

Use of silicone implants for breast augmentation is a safe technique, however, there are some women who object to carrying a foreign body for aesthetic purposes. Mesenchymal cell (previously known as stem cell) enriched fat injection offers such women an alternative to silicone implants. Normally, in fat injections, around 60 % of injected fat is resorbed after a few months. Addition of mesenchymal cells to fat aspirates, decreases fat resorption to much lower levels and makes it possible to transfer large amounts of fat in one session. Mesenchymal cells are harvested from fat with an FDA approved device. This technique is suitable for patients who already have some breast tissue and wishing only a moderate augmentation; who have adequate fat available in the body for aspiration, and who do not have fibrocystic disease or lesions with a malignant potential. Therefore a preoperative mammography is required.

Breast Lifting

Breasts may sag due to excessive weight loss, child birth or aging. It is possible to lift the breasts without changing their size. This procedure requires skin excision, so there will be scars. Scar patterns may differ from one that is only around the nipple-areola complex to a classical inverted T-shape. This is related to the amount of drooping.

Breast Lifting with Augmentation

Breasts drooping related to excessive weight loss, child birth or aging may also cause loss of soft tissues in the breasts. In those cases lifting will not be sufficient to obtain an aesthetically pleasing result. Then a silicone implant may be used in combination with lifting surgery to have well-shaped and projecting breasts. The implant shape recommended is the anatomical one. The scars will be similar to those in breast lifting.

Breast Reconstruction

Breast cancer is very common and happens in every 8th woman. Loss of a breast is nothing less than loss of an organ or extremity. It can be restored with several surgical techniques and may look almost identical to the native breast except the scars. Nipples and areolas are also reconstructed but usually in another session 3 to 6 months later.

The best method for breast reconstruction is with one's own tissue from the belly fat. It is now possible to transfer belly fat that is resected as if in a tummy-tuck procedure to the breast with microsurgical techniques. If the belly fat is not available breast can be reconstructed with either tissue transferred from the back or buttocks.

For smaller breasts or in cases where both breasts are lost, silicone breast implants may also be used. However, silicone implants are not advised if the patient had radiotherapy. If silicone implants are used, it requires a two-stage procedure that is 3 months apart.

The breast reconstruction can be done immediately during the cancer resection or later. If it is done later, it should be postponed until after chemotherapy and radiotherapy.
