What is cataract?

Eye is a system which is similar to a camera. The light and image coming from outside have to fracture firstly in the cornea which is the outer part of the eye, later in the lens layer which is the inner part of the eye in order to reach the sight centre. Normally, all these layers have transparent structure. Cataract is a condition that the lens in the eye loses its transparency and becomes opaque.

Although it has got many types, generally, cataracts are divided in 3 main groups; 1. Cataracts due to the age 2. Congenital Cataracts 3. Secondary Cataracts (They occur according to the usage of pills for a long time like cortisone, impacts, metabolic illnesses like diabetes.)

Cataract mostly occurs due to the age. Although its main reason is not known, it has got many other risk factors like diet, ultraviolet rays. Due to the opaqueness of the lens, patients firstly complain from the far or near sight loss. As the opaqueness increases, both far and near sight loss affect the patients’ social life.

When the sight problem occurs in a patient, an operation is done. There is no need to wait. Anticipation complicates the operation.

There is no drug therapy for cataract. The only cure for it is an operation. Cataract can not be eradicated by any pills, drip medicine, exercise, or eye glasses.

The only way to cure it is to replace the transparent inner eye lens with the lens in the eye which has cataract. With the improvement of the modern techniques, cataract operations are done without pins, narcosis, and pain. The operation is done by drip medicine which anaesthetizes the eye and the patient might return to home on the same day.

• The far or near sight function might fall off or forfeit.
• The eye tension might rise.
• Amblyopia might flourish.
• Inflamations like uveitis might evolve.
• Thediagnosis and cure of problems in the inner eye might get difficult because the sight centre and retina cannot be monitored during the consultation.

Cataract does not recur. There might be opaqueness in the capsule in which the inner eye lense is replaced after an operation which is done by lenses which are not approved by FDA. This opaque layer is opened by a shredder laser called YAG Laser.

However, this is an application which is done after an operation that it is not an operation. The patient might return to daily activities after YAG Laser application.

The patient who is planning to have an operation must talk to the doctor in details about the lens be replaced. If they request a multifocal inner eye lens which shows far and near without eye glasses, they must tell it to the doctor because most of the doctors do not use a multifocal lens if they are not mentioned especially. So they use a standard multifocal lens. In this situation, the patients lose the advantage to see near without glasses. The multifocal lenses we use are a bit more expensive than the standard ones. So that, the operations we do are a bit more expensive too.

Now, reducing the lens of eyesight and vision It can be treated with laser cataract treatment without touching. Instead of classic knife surgery, femtosecond laser is used in cataract eye-surgery with a new era technology. Thanks to the new technology ,postoperative risks and unexpected side effects is coming down the minimum. The doctor who performed the surgery with ‘Femtosecond laser technology ‘ is installing incision of the shape of the eye cornea, angle and size on the computer. Then, s/he enters the required data with opening capsule containing cataract. Next s/he is installing the required values for the decomposition of the cataract.

After all, It remains to be done the laser operation by pressing the device of the laser. Small defects in astigmatic cornea can be corrected with laser which called ‘arcuate’.

Femtosecond laser cataract surgery patients can go home after 15 minutes operation. Again, they heal quickly and return to their daily life in a short time.

• Before surgery, consult with your doctor about aspirin and similar blood thinning drugs.
• On the day of surgery; diabetes, hypertension, asthma and like these drugs used for systematic diseases should not be taken.
• Continue to follow a diet ,insulin dose and oral antidiabetic treatment as usual.
• The day of surgery, bring your medications systematically .
• Wash your face thoroughly and do not make up, don’t use lotion and perfume the day of surgery.
• Keep a companion with you.
• Do not rub the eye after surgery and avoid applying pressure.
• Use protective eye bandage and sunglasses if you go outside windy and sunny weather.
• Use medication regularly as recommended by doctor.
• If necessary ,eyeglasses are prescribed by doctor.

While some people use separate glasses for both close and far, some people can see both close and far with only a pair of glasses. The multifocal lenses are intraocular lenses which ensure seeing both close and far.

Selection of patient is crucial for this treatment. It is significant to see whether far or close for the patient, it should be paid attention. Another important topic in selection of patient is the pupil dimensions of the patient.

• The ones who have high myopia, hypermetropia or astigmatic which cannot be improved with excimer laser.
• The ones whose cornea thickness does not allow the laser.
• The patients who have cataract and high refraction defect
• The patients whose eye number is stable, not proceeding

Sure, it is essential that the patient should be undergone detailed examination. The eye of each patient can not be appropriate for this kind of operation. The occupational status, age, social activity, reading habit and intellectual level of patients who have cataract have a place in selection of patient, too.
